Job Summary
Deliver individualized, one-on-one nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide skilled interventions including wound care, infusions, and medication management while educating patients and caregivers on symptom management. Monitor patient progress closely, communicate with physicians to adjust plans, and coordinate care across providers to ensure continuity. Complete timely, accurate documentation reflecting the full clinical picture. Schedule includes Monday through Friday daytime visits with a shared on-call rotation for Strum, Eleva, Osseo, Fairchild, and surrounding communities. Requires an active RN license with one year of clinical experience, a current driver's license, and CPR certification.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Current driver's license
- Reliable transportation
- Auto insurance
- CPR certification (or ability to obtain certification)
- Compliance with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons
