Job Summary
Deliver one-on-one in-home nursing care as a primary clinical partner, including comprehensive in-home assessments, wound care, infusions, catheter care, medication management, post-operative and chronic disease management, and education to patients and caregivers. Coordinate care with the interdisciplinary team, monitor progress, adjust plans to promote safety and independence, and complete timely documentation to support quality outcomes. This Full-Time Home Health RN role serves patients in Fall Creek, Augusta, Osseo, Fairchild and surrounding communities, with an on-call rotation and a focus on patient-centered care.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least 1 year of clinical nursing experience
- Current driver’s license, reliable transportation, and auto insurance
- CPR certification (or ability to obtain certification)
