Full-Time Home Health Physical Therapist - Western Physical Therapy,Rochester,NY

Conduct comprehensive patient assessments to evaluate functional ability, develop personalized treatment plans, and deliver direct care through individualized therapy sessions. Instruct patients, caregivers, and families on therapeutic exercises and assistive device use to promote independent living. Prioritize caseload and coordinate daily assignments with a multidisciplinary team while maintaining detailed documentation to track progress and adjust plans as needed. Work onsite within home health care settings serving diverse adult and pediatric populations, including post-surgical and chronic condition rehabilitation. Requires a valid New York Physical Therapy license, DPT degree, and BCLS certification within 30 days.
