Job Summary
Conduct comprehensive assessments to formulate personalized treatment plans for patients referred by physicians. Implement therapy sessions using evidence-based methodologies to promote healing and functionality. Educate patients and caregivers regarding conditions and strategies for effective home exercise regimens. Document patient progress and communicate findings with interdisciplinary teams to drive optimal care outcomes. Adhere to state regulations and ethical standards while maintaining professional development through continued education and team meetings. Work onsite in patient homes Monday through Friday, focusing on orthopedic, pediatric, and sports injury cases.
Required Qualifications
- Bachelor's Degree in Physical Therapy from an APTA or CAPTE-approved program
- Current state licensure in Physical Therapy
- Basic Life Support (BLS) certification from the American Heart Association
- Effective written and verbal communication skills
- Proficiency in word processing
- Familiarity with electronic health records (EHR) systems
- Strong interpersonal skills
- Minimum of two years of clinical experience
- Ability to work onsite
- Availability for Monday through Friday schedule
