Full Time Home Health Physical Therapist (PT)

Evaluate patients referred by physicians using diagnosis-specific assessments and provide in-home PT visits for adult and geriatric patients. Complete timely documentation including evaluations, progress notes, and time and mileage records. Schedule and manage daily patient visits while collaborating with physicians and interdisciplinary team members to communicate patient progress and updates. Drive between patient homes with mileage reimbursement, noting most patients are close together with occasional 20–40 minute drives. Full-time salary with very competitive pay and 5+ weeks of PTO annually.
