Job Summary
Evaluate patient conditions through assessments and interviews to develop personalized treatment plans based on physician referrals. Implement therapy programs utilizing manual therapy, exercise prescription, and modalities to improve functionality and reduce pain. Provide education and training to patients and caregivers regarding rehabilitation processes and injury prevention strategies. Document patient progress in compliance with healthcare regulations and collaborate with interdisciplinary teams to optimize care outcomes. Work primarily in patients' residences to deliver personalized interaction and comprehensive understanding of living conditions. Concentra Rehabilitation Center is a physical therapist-owned private practice fostering a holistic approach to recovery across the United States.
Required Qualifications
- A Bachelor's Degree in Physical Therapy or a Doctorate in Physical Therapy (DPT) from an accredited institution
- A valid Physical Therapy license in the state of Florida
- Certification in Basic Life Support (BLS) issued by the American Heart Association
- Strong communication skills for effective interaction with patients and their families
- Proficient computer skills and capability to document effectively using electronic health record systems
Desired Qualifications
- A genuine desire to build lasting relationships with patients and support them through their rehabilitation journey
- A minimum of two years of relevant experience in a physical therapy setting
- Experience in home health or similar settings
