Job Summary
Conduct thorough evaluations of patients to assess physical function, mobility, and pain levels, then design and implement personalized rehabilitation plans tailored to individual recovery goals. Administer manual therapy techniques and exercise programs while setting measurable progress targets and monitoring outcomes to adjust treatment strategies as needed. Educate patients and families on home exercises and lifestyle modifications, and collaborate with physicians and occupational therapists to develop comprehensive care plans. Document all patient progress accurately in compliance with state regulations and organizational policies. Visit patients in their homes Monday through Friday from 9:00 am to 5:00 pm to provide direct therapeutic services to geriatric clients, post-surgical patients, and those with chronic conditions.
Required Qualifications
- New York State Physical Therapist License
- Bachelor's degree in Physical Therapy or a related field
- Minimum of two years of experience as a Physical Therapist
