Job Summary
Conduct thorough examinations and evaluations of patients with various functional limitations and disabilities. Develop customized treatment plans based on individual patient assessment, goals, and progress. Implement treatment modalities, educate patients and families about the rehabilitation process, and promote adherence to treatment plans. Collaborate closely with other healthcare professionals and support staff, ensuring a holistic approach to patient care. Maintain accurate documentation regarding patient treatment, progress, and discharge planning in compliance with regulatory standards. Participate in community outreach initiatives and professional development opportunities, including workshops and training sessions. Supervise assigned support staff and help manage department resources efficiently. This full-time role requires onsite clinical visits to multiple patients in South River, New Jersey, with a salary range of $100,000 to $120,000.
Required Qualifications
- A Bachelor's Degree in Physical Therapy from an accredited institution
- A valid and active State License in Physical Therapy
- Basic Life Support (BLS) certification
- Strong written and verbal communication skills
- Proficiency in electronic record keeping
- Word processing skills
- Familiarity with healthcare software systems
- Minimum of two years of experience as a Physical Therapist
- Must be able to work onsite in the home care setting within South River, New Jersey
- Must comply with vaccination requirements
- Must be able to lift 50 lbs
