Job Summary
Conduct thorough evaluations to assess patients' physical abilities, limitations, and goals. Develop tailored treatment plans based on evaluations and physician referrals, incorporating the latest evidence-based practices. Implement therapeutic interventions including exercise programs, manual therapy techniques, and patient education. Monitor and document patient progress, adjusting treatment plans as necessary. Provide guidance and support to patients and caregivers regarding conditions, treatment plans, and home exercise programs. Collaborate with a multidisciplinary team to create comprehensive care plans. Work Monday through Friday with a standard 40-hour week, delivering care in patients' homes across diverse demographics including orthopedic, pediatric, neurologic, and sports medicine cases.
Required Qualifications
- A Bachelor's degree in Physical Therapy from an accredited program
- A valid State License to practice Physical Therapy
- Certification in Basic Life Support from the American Heart Association
- Excellent verbal and written communication skills to connect effectively with patients and their families
- Competency in using technology and electronic health record systems to document care
- A commitment to building strong patient relationships to help them achieve their functional goals
Desired Qualifications
- A Doctorate of Physical Therapy (DPT) is preferred, though not required
- At least two years of clinical experience in a relevant setting, demonstrating expertise in diverse therapeutic modalities
- Current CPR certification
