Job Summary
Conduct comprehensive patient evaluations to determine physical capabilities and limitations, then develop, implement, and modify individualized treatment plans based on findings and patient feedback. Consult with healthcare providers to coordinate ongoing care and ensure effective treatment transitions while providing educational support to patients and families regarding the treatment process and discharge plans. Document all clinical activities, including evaluations, sessions, and progress, adhering to regulatory compliance standards. Participate in interdepartmental meetings to discuss patient outcomes and share best practices, maintaining effective communication with interdisciplinary teams, physicians, and external referral sources. Contribute to community outreach and quality improvement initiatives that enhance service delivery and program development.
Required Qualifications
- A Bachelor's Degree in Physical Therapy from an accredited institution
- A current state license in Physical Therapy
- Basic Life Support (BLS) certification
- Excellent verbal and written communication skills
- Proficiency in electronic medical record (EMR) systems and documentation practices
- A minimum of 2 years of experience in physical therapy
- Must be available for weekend shifts
- Must adhere to a drug-free workplace
- Must adhere to mandatory vaccinations
