Job Summary
Conduct thorough evaluations of patients' physical capabilities and develop individualized treatment plans based on clinical assessments. Provide consultations to healthcare providers regarding patient progress and maintain compliance with all applicable laws while documenting outcomes. Educate patients and families on treatment plans and collaborate extensively with interdisciplinary teams to ensure integrated care. Supervise and delegate tasks to assigned support staff, manage departmental resources, and participate in leadership initiatives including community outreach and program development. Maintain a flexible schedule including weekends with 40 scheduled hours per week.
Required Qualifications
- A Bachelor's Degree in Physical Therapy from an accredited program
- A valid state license to practice Physical Therapy in New Jersey
- Basic Life Support (BLS) certification
- Excellent verbal and written communication skills
- Proficiency in word processing and electronic health record (EHR) systems
- Working onsite in the homes of our patients
- A flexible schedule, including weekends
- 40 scheduled hours per week
