Job Summary
Conduct thorough evaluations of patients to identify functional limitations and develop comprehensive treatment plans tailored to individual needs. Implement evidence-based treatments, re-evaluate progress regularly to modify plans, and educate patients and families about conditions and recovery processes. Consult with healthcare providers to ensure coordinated care and collaborate with interdisciplinary teams regarding treatment changes and discharge planning. Ensure accurate documentation of evaluations, treatments, and outcomes while adhering to relevant laws and regulations. Participate in community outreach initiatives and guide less experienced staff through clinical education opportunities. This full-time role in Danbury, Connecticut, serves patients managing chronic conditions or recovering from surgeries in their homes.
Required Qualifications
- A Bachelor's Degree in Physical Therapy from an accredited institution
- A valid Physical Therapy State License
- Basic Life Support (BLS) certification
- Strong written and verbal communication skills
- Proficiency in using technology for documentation and care coordination
- Must be able to lift 50 lbs
- Must be available for weekend shifts
- Valid driver's license
