Job Summary
Evaluate patients thoroughly and craft individualized treatment plans in collaboration with referring physicians. Implement a wide range of therapeutic interventions in patient homes using evidence-based techniques, while monitoring and documenting progress to adjust strategies as needed. Provide education to patients and caregivers regarding home exercise programs and self-management strategies, ensuring superior customer service throughout the treatment journey. Collaborate with fellow healthcare professionals to optimize outcomes and achieve utilization goals while maintaining the highest standards of care for diverse populations including orthopedic, pediatric, and neurologic conditions.
Required Qualifications
- A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
- A valid State License as a Doctor of Physical Therapist (DPT) issued by the Physical Therapy Examining Committee.
- A minimum of two years of relevant clinical experience.
- Current CPR certification.
- Exceptional communication skills, both written and verbal, to effectively connect with patients and caregivers.
- Proficiency in computer systems and documentation tools to streamline patient management.
- A strong desire to foster positive relationships with patients to achieve and surpass their recovery goals.
Desired Qualifications
- Experience in home healthcare settings or familiarity with diverse patient demographics will be advantageous.
