Job Summary

Manage discharge planning and maintain accurate documentation of patient conditions and care continuity. Ensure effective time management by meeting productivity expectations and participating in community outreach. Maintain a consistent schedule across Bridgeport and surrounding areas while pursuing continuing education and mentoring students.
Required Qualifications
- A Bachelor's Degree
- Valid and active State License in Physical Therapy
