Job Summary
Conduct comprehensive patient evaluations and develop individualized treatment plans for orthopedic, pediatric, neurological, and sports medicine cases. Provide direct therapeutic interventions in patients' homes while educating caregivers on exercises and lifestyle adjustments to promote independence. Maintain evidence-based practice standards, actively participate in team meetings, and contribute to quality improvement initiatives to optimize patient outcomes.
Required Qualifications
- A Doctorate Degree in Physical Therapy (DPT)
- A valid State License in Physical Therapy
- At least two years of clinical experience in diverse physical therapy settings, especially home care
- Basic Life Support (BLS) Certification from the American Heart Association
- Proficient written and verbal communication skills
- Strong computer skills, including word processing, documentation, and familiarity with electronic health records
- A commitment to building meaningful relationships with patients to encourage engagement in their rehabilitation journey
- A genuine desire to connect with individuals and their families to understand their needs and aspirations
- A collaborative mindset and experience working within a community-oriented health care environment
