Job Summary
Conduct comprehensive home evaluations to assess patient needs and develop individualized treatment plans. Implement therapeutic interventions including exercise regimens, manual therapy, and specialized equipment to alleviate pain and improve mobility. Systematically document treatments, responses, and progress while monitoring goals to adjust plans as patient conditions change. Advise patients and families on assistive device usage and self-care strategies to enhance independence. Engage in ongoing professional development through workshops and society participation to maintain high practice standards. Work onsite in patient homes Monday through Friday, collaborating with a close-knit team to ensure seamless care for diverse demographics recovering from injuries, surgeries, or chronic conditions.
Required Qualifications
- Bachelor's degree in Physical Therapy from an accredited institution
- Valid state license to practice as a Physical Therapist
- Strong verbal and written communication skills
- Proficiency in computer applications related to documentation and treatment planning
- Detail-oriented and able to work independently with a high degree of flexibility and adaptability
- Current CPR certification
Desired Qualifications
- A minimum of two (2) years of experience in a physical therapy or rehabilitation setting, especially in home health care
- Experience in developing treatment plans and a proven track record in improving patient rehabilitation outcomes
- Familiarity with assistive technology and adaptive devices
