Job Summary
Develop and execute personalized treatment plans to prevent disability, relieve pain, and enhance mobility for elderly clients recovering from surgery, those with chronic pain, and individuals rehabilitating from injuries. Oversee Physical Therapy Assistants by guiding and supervising their implementation of care plans while conducting thorough evaluations through diagnostic assessments and home environment analyses. Administer therapeutic interventions including manual exercises, traction, and massage, and complete detailed clinical documentation for initial evaluations, progress notes, and discharge summaries. Collaborate with interdisciplinary team members during case conferences to adjust strategies based on observed condition changes and educate clients on device usage.
Required Qualifications
- A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
- A valid, current state license to practice as a Physical Therapist in Florida.
- Current CPR certification
- A valid driver's license with automobile insurance
- Strong interpersonal skills and the ability to build rapport with patients and their families
- A proactive mindset, capable of making informed decisions and advocating for patients' needs
- Minimum of two years of clinical experience
Desired Qualifications
- Experience in home care or acute care settings
- Familiarity with federal, state, and governing agency regulations governing home healthcare practices
