Job Summary
Direct and implement physical therapy treatments to relieve pain, restore function, and prevent disability based on each client's unique needs. Evaluate and diagnose clients' functional abilities through muscle, nerve, joint, and functional assessments. Design and propose modifications to rehabilitation plans while advising families to ensure alignment with overall care strategies. Oversee and mentor Physical Therapy Assistants to guarantee accurate adherence to care plans. Document clinical records, including initial evaluations, care plans, progress notes, and discharge summaries, ensuring regulatory compliance. Administer therapeutic exercises, traction, and other physical modalities to assist clients in achieving rehabilitation goals. Collaborate with an interdisciplinary team for case management and strategize individualized treatment approaches through regular case conferences. Engage with clients and families to educate them on therapeutic programs and assistive devices, fostering independence. Serve pediatric to geriatric patients requiring rehabilitation due to recent surgeries, chronic pain, or debilitating conditions.
Required Qualifications
- A Doctorate in Physical Therapy (DPT) from an accredited institution
- A valid Physical Therapy license in the state of Florida, in good standing
- Current CPR certification
- A valid driver's license with automobile insurance
- Effective communication skills, demonstrating expertise in patient engagement
- A passion for delivering high-quality, patient-centered care within a home care setting
Desired Qualifications
- At least two years in physical therapy
- A background in acute care or home care settings
- Proven ability to mentor and guide Physical Therapy Assistants
- Experience in managing complex cases
