Job Summary
Assess and evaluate patients recovering from injuries, surgeries, or chronic conditions directly in their homes. Develop individualized treatment plans utilizing diverse therapeutic modalities, including manual therapy, tailored exercise programs, and state-of-the-art rehabilitation equipment. Implement interventions to promote increased mobility and pain relief while maintaining thorough documentation of evaluations and progress notes. Provide education to patients, caregivers, and families on effective use of assistive devices and best practices for rehabilitation. Engage in continuous learning through professional workshops and training sessions to keep abreast of latest practice standards.
Required Qualifications
- A Doctorate of Physical Therapy (DPT) from an accredited institution.
- Active State license to practice as a Doctor of Physical Therapist (DPT).
- A minimum of two (2) years of clinical experience in physical therapy, ideally with a focus on orthopedic rehabilitation.
- Current CPR certification.
- Strong communication skills to effectively interact with patients and healthcare teams.
Desired Qualifications
- Experience working in a home health environment and familiarity with assistive technology and devices.
- Additional certification in specialized therapeutic areas, such as manual therapy, sports rehabilitation, or geriatric physical therapy.
