Full-Time Geriatric Physical Therapist

Job Summary
Conduct thorough evaluations of patients' physical abilities to establish individualized treatment plans for geriatric clientele. Deliver skilled therapy services including therapeutic exercises, manual therapy, and various modalities while continuously monitoring and documenting progress to adjust plans based on evolving needs. Collaborate effectively with physicians, occupational therapists, and speech therapists to provide a holistic approach, and educate patients and families on therapeutic techniques and home exercise regimens. Implement fall prevention programs and safety measures to mitigate risks within the assisted living community. Work onsite Monday through Friday from 9 am to 5 pm in a collaborative outpatient facility dedicated to empowering seniors to achieve optimal health and functionality through successful aging-in-place.
Required Qualifications
- A graduate from an accredited college or university with a Bachelor's, Master's, or Doctorate degree in Physical Therapy
- Possession of an active State license to practice as a Physical Therapist
- Demonstrated knowledge of evidence-based practices in physical therapy
- Strong interpersonal skills capable of building rapport with patients, families, and medical professionals
- Proficiency in using electronic medical records (EMRs) and documentation systems relevant to patient care
- A commitment to providing patient-centered care focused on improving the quality of life for all patients
- Ability to work both independently and as part of a multidisciplinary team, demonstrating effective collaboration to enhance patient outcomes
- Onsite work in Elizabethport, New Jersey
- Monday to Friday schedule from 9 am to 5 pm
Desired Qualifications
- Previous experience in geriatric therapy
- Previous experience with electronic medical records
