Job Summary
Conduct thorough evaluations of patients' physical abilities to establish personalized treatment plans tailored to their unique needs and goals. Deliver skilled therapy services, including therapeutic exercises, manual therapy techniques, and utilization of state-of-the-art modalities in treatment sessions. Regularly monitor and document patient progress, updating treatment plans to ensure optimal outcomes based on observed improvements or challenges. Collaborate closely with a multidisciplinary team of healthcare professionals, including physicians and other therapists, to provide comprehensive care that addresses all aspects of the patient's health. Educate patients and their families about therapeutic techniques, home exercise programs, and strategies for injury prevention. Implement fall prevention programs and safety measures, ensuring that our geriatric clientele is safeguarded against potential accidents and injuries. Stay abreast of the latest advancements in physical therapy through ongoing education and professional development. Work onsite Monday through Friday from 9 AM to 5 PM.
Required Qualifications
- A graduate from an accredited college or university holding a Bachelor's, Master's, or Doctorate degree in Physical Therapy
- Current and active State licensure as a Physical Therapist
- Strong interpersonal and communication skills necessary for effective interaction with patients, their families, and the wider healthcare team
- Familiarity with electronic medical records (EMRs) and relevant documentation systems, enabling efficient patient data management
- Knowledge of evidence-based practices in physical therapy and the capacity to implement cutting-edge therapeutic techniques
- A strong commitment to delivering patient-centered care that prioritizes enhancing patient independence and overall quality of life
- Demonstrated ability to work effectively both independently and collaboratively within a multidisciplinary team
- Prior experience in geriatric therapy
- Experience in developing and implementing individualized treatment plans for older adults
