Job Summary
Conduct comprehensive in-home wellness visits and health assessments for members with chronic conditions, including medical history reviews, physical examinations, vital signs collection, and preventive screenings such as spirometry and bone density tests. Deliver preventive health education, manage medication reconciliation, and collaborate with the care team to identify diagnoses for risk adjustment documentation and close quality care gaps. Maintain accurate EMR documentation in compliance with HIPAA regulations while supporting informed decision-making and goals of care discussions. Full-time role covering the entire state of Oregon with daily travel within a 55-mile radius.
Required Qualifications
- Active, unencumbered Nurse Practitioner (NP) license in Oregon
- Minimum 3 years of direct patient care experience
- Full-time employment in Marion County, Oregon
- Daily travel within a 55-mile radius of assigned home location
- Statewide travel availability with minimum two weeks' notice
- HIPAA compliance and PHI safeguarding
- Accurate, timely, and compliant EMR documentation
