Job Summary
Evaluate, diagnose, and treat patients across the lifespan while developing individualized treatment plans and managing ongoing care. Prescribe medications, coordinate referrals, and maintain accurate electronic medical record documentation. Collaborate with physicians, specialists, therapists, and other healthcare providers to build and manage a thriving patient panel. Utilize telemedicine when appropriate and contribute to a culture of excellence and continuous improvement.
Required Qualifications
- Licensed Family Nurse Practitioner (FNP) or Physician Associate / Physician Assistant (PA)
- Current Illinois licensure
