Job Summary
Provide compassionate physical therapy and wellness services to older adults under the supervision of a licensed Physical Therapist. Support care coordination through regular wellness check-point visits, assessing progress and communicating with patients, families, and healthcare providers. Facilitate access to telehealth services by assisting patients in attending virtual visits and receiving necessary therapeutic interventions. Maintain high professional standards by adhering to company policies, ethical guidelines, and clinical initiatives. Engage in program development initiatives and quality improvement projects to enhance service delivery and patient outcomes. Work alongside therapists to optimize each patient's functional well-being and satisfaction through dedicated, personalized care.
Required Qualifications
- A completed accredited Physical Therapy Assistant program
- Current state licensure as a Physical Therapist Assistant in Colorado
- Exceptional verbal and written communication skills
- A results-oriented approach coupled with strong critical thinking abilities
- Technological proficiency and adaptability
- A genuine curiosity for learning and improving clinical skills
Desired Qualifications
