Job Summary
The position requires providing one-on-one in-home therapy for post-operative patients, devising and executing detailed treatment plans, maintaining positive relationships with clients and staff, working independently while providing empathetic patient care, and ensuring compliance with all state and federal laws. Ideal candidates will possess a Bachelor's Degree in Physical Therapy, current licenses in Indiana and Illinois, and have at least one year of preferred experience in an outpatient setting. Required Qualifications
- Bachelor’s Degree from an accredited Physical Therapy Program.
- Current State of Indiana and Illinois Physical Therapy licenses.
- Valid Driver’s License, reliable transportation, and required liability insurance to perform home visits.
Desired Qualifications
- Minimum one-year experience in outpatient setting preferred.
- Current CPR Certification.
- BCLS or ACLS certification.
