Job Summary
PT - Physical Therapist provides direct rehab care in physical therapy to patients upon physician referral in accordance with all applicable laws, regulations, and Life Care standards. Collaborates with an interdisciplinary team to support residents in a skilled nursing facility setting, overseeing PTAs and students, and implementing treatment plans to improve functional progress and quality of life. Must be a graduate of an accredited PT program (BSPT, MSPT, or DPT); new grads welcome. Proficient with Microsoft Word/Excel/email; able to communicate in English; dedicated to patient rights, safety, confidentiality, and compliance with Life Care policies. Schedule is Monday–Friday with a sign-on bonus and comprehensive benefits; responsibilities include directing rehab care, coordinating with nursing and medical staff, and maintaining documentation using therapy software.
Required Qualifications
- Graduate of an accredited program in physical therapy, BSPT, MSPT, or DPT (2003 or later must have MSPT or DPT)

- Read, write, speak, and understand the English language
- Proficient in Microsoft Word, Excel, and email
- Follow physical therapy treatment plans for patients
- Oversee and evaluate care given by PTAs, Rehab Aides, and PT and PTA students
- Utilize therapy software appropriately and accurately
