Job Summary
As a PT for Life Care Centers of America facilities, provide direct rehab care in physical therapy to patients upon physician referral within SNFs and post-acute settings. Collaborate with an interdisciplinary team of nursing, therapy, and medical staff to support functional progress and quality of life. Oversee and evaluate care by PTAs and Rehab Aides, use therapy software accurately, and maintain patient rights, safety, confidentiality, and compliance with policies. Requires graduation from an accredited physical therapy program (BSPT/MSPT/DPT) and proficient communication in English; new grads are welcome. Familiarity with Microsoft Word/Excel, and Liaison activities with patients and families are valued. Read, write, speak, and understand English; demonstrate professionalism and adherence to Life Care’s Code of Conduct and compliance training.
Required Qualifications
- Graduate of an accredited program in physical therapy, BSPT, MSPT, or DPT (2003 or later must have MSPT or DPT)
