Job Summary
Provide one-on-one physical therapy to older adults in their homes or within senior living communities across Long Island, NY, delivering in-home, in-community, and telehealth care to improve mobility, independence, and functional outcomes. Schedule autonomy with salaried positions offering benefits; collaborate with an interdisciplinary PT/OT/SLP team; document in an EMR; travel to patient homes and senior living sites as part of the Geriatric House Calls model; telehealth/virtual care options expand access. Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Possess or be eligible for a Physical Therapy license in NY (or applicable state)
- In-home and senior living care experience preferred
- Ability to drive to patient homes and communities
