Job Summary
Deliver one-on-one physical therapy to older adults in private residences, senior living communities, and memory care settings to improve mobility and functional independence. Document sessions effectively within FOX's EMR system and collaborate with an interdisciplinary team including OTs, SLPs, and fitness specialists. Incorporate telehealth and virtual care components to extend access and support continuity of care. Drive to patient homes and community partners as part of the Geriatric House Calls model, providing moderate to maximum assistance with transfers and bed mobility. This role supports independence and wellness for seniors across Clackamas and surrounding areas within a clinician-operated practice rooted in evidence-based care.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities as part of the Geriatric House Calls model
- Must be available for in-home, senior living, and virtual care to older adults across Clackamas and surrounding communities
