Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities, supporting mobility, independence, and functional longevity. Deliver sessions that incorporate telehealth or virtual care when appropriate to extend access and flexibility. Document effectively within FOX's EMR system while collaborating with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists. Manage your own treatment schedule to create autonomy and build lasting patient relationships. This role supports a mission to abolish ageism within a clinician-operated practice rooted in evidence-based care.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities as part of the Geriatric House Calls model
