Job Summary
Provide one-on-one physical therapy to older adults in private residences and senior living communities, supporting mobility, independence, and functional wellness. Deliver sessions that include moderate to maximum assistance with patient transfers and bed mobility, while documenting effectively within FOX's EMR system. Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists, and incorporate telehealth or virtual care when appropriate to extend access and flexibility. Work within a clinician-operated practice rooted in meaningful relationships and evidence-based care, utilizing advanced technology and tools to help patients live stronger, longer lives.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities as part of the Geriatric House Calls model
Desired Qualifications
- Experience with telehealth and other forms of virtual care
