Job Summary
Provide one-on-one physical therapy to older adults in private residences, senior living communities, and via telehealth to improve mobility and functional independence. Document sessions effectively within the EMR system and collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists. Manage your own treatment schedule while meeting physical demands such as patient transfers and driving to patient locations. Full-time and part-time salaried roles with sign-on bonuses, H1B sponsorship, and benefits are available for licensed clinicians in Rocklin, CA.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities as part of the Geriatric House Calls model
