Job Summary
Provide one-on-one physical therapy to older adults in private residences, senior living communities, and memory care settings to improve mobility and functional independence. Deliver sessions supporting patient wellness and longevity while documenting effectively within FOX's EMR system according to regulatory standards. Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists, incorporating telehealth and virtual care when appropriate to extend access and flexibility. Drive to patient homes and communities as part of the Geriatric House Calls model, meeting physical demands including moderate to maximum assistance with transfers and bed mobility.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities as part of the Geriatric House Calls model
Desired Qualifications
- Experience with telehealth and other forms of virtual care
