Job Summary
Provide one-on-one physical therapy to older adults in private residences, senior living communities, and memory care settings within Thibodaux, LA. Deliver sessions supporting mobility, independence, and functional longevity while documenting effectively in FOX's EMR system. Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists, and incorporate telehealth or virtual care when appropriate to extend access. Manage your own treatment schedule with autonomy, providing moderate to maximum assistance with patient transfers and bed mobility. Drive to patient homes and communities as part of the Geriatric House Calls model.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities as part of the Geriatric House Calls model
Desired Qualifications
- Experience with telehealth and other forms of virtual care
