Job Summary
Provide in-home and senior living physical therapy to older adults, delivering one-on-one sessions in private residences and senior living communities. Develop treatment plans to improve mobility, independence, longevity, and functional outcomes; document effectively within FOX’s EMR in accordance with regulatory standards; collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists); incorporate telehealth/virtual care when appropriate to extend access and flexibility; drive to patient homes and work within the Geriatric House Calls model to support patients across Marysville and surrounding communities.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
