Job Summary
Provide and coordinate Physical Therapy services for residents in a skilled nursing setting, including assessment, treatment, program planning and implementation, related documentation, and communication. Operate under physicians' orders, conduct regular resident screenings to determine intervention needs, evaluate residents for treatment planning, and initiate referrals when appropriate. Must hold a Bachelor of Science in Physical Therapy and be a National Certification Examination–registered Physical Therapist, with 0–3 years of long-term care experience; comply with AHCA background screening.
