Job Summary
Provide one-on-one physical therapy to older adults in home-based settings (private residences, senior living communities, and other community sites) with a focus on mobility, independence, and functional outcomes. Roles include delivering in-home and senior-living care sessions, documenting in FOX’s EMR, collaborating with an interdisciplinary team (PT, OT, SLP, Fitness Specialists), and optionally incorporating telehealth/virtual care to extend access. Candidates should be licensed as a Physical Therapist (or eligible to obtain licensure) and demonstrate strong patient relationship-building, autonomy in scheduling, and the ability to drive to patient homes as part of the Geriatric House Calls model. Required Qualifications
