Job Summary
Provide one-on-one physical therapy to older adults in private residences or senior living communities, supporting mobility, independence, and functional goals through the Geriatric House Calls model. Responsibilities include delivering therapy sessions in-home or in senior living settings, documenting in FOX’s EMR, collaborating with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists), and offering telehealth/virtual care when appropriate to extend access. Must be licensed (or eligible to obtain license) in the practicing state, complete reports in compliance with regulatory standards, and drive to patient homes and communities to deliver in-home and community-based care. Opportunities include full-time, part-time, and PRN/Flex roles with benefits, H1B sponsorship available for qualified clinicians, and a mentor program for new graduates.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
