Job Summary
Provide one-on-one physical therapy to older adults in private residences, senior living communities, and memory care facilities. Deliver sessions supporting mobility, independence, and functional wellness while documenting effectively within FOX's EMR system. Collaborate with interdisciplinary teams including OTs, SLPs, and fitness specialists. Incorporate telehealth and virtual care components to extend access and flexibility. Manage your own treatment schedule with autonomy in a clinician-operated practice. Drive to patient homes and communities as part of the Geriatric House Calls model.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to provide moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities
Desired Qualifications
- Experience with telehealth and other forms of virtual care
