Job Summary
Provide one-on-one physical therapy to older adults in private residences, senior living communities, and via telehealth to improve mobility and functional independence. Document sessions effectively within FOX's EMR system and collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists. Manage your own treatment schedule while delivering moderate to maximum assistance with patient transfers and driving to client locations. This clinician-operated practice supports independence and wellness across Hanahan and surrounding communities, offering optional virtual care components and an Emerging Professionals Mentor Program for new graduates.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to meet the physical demands of in-home and community-based care, including: Driving to patient homes and senior living communities as part of the Geriatric House Calls model

- Experience with telehealth and other forms of virtual care
