Job Summary
Plan and administer prescribed skilled physical therapy treatment and training for patients in home health, focusing on rehabilitation and functional improvement. Responsibilities include testing patient strengths, developing and implementing therapy plans in collaboration with physicians and clinical team, supervising staff, documenting care observations and progress, and communicating patient status to physicians, supervisors and families. Requires a degree from an accredited Physical Therapy program and an active PT license; one year experience preferred. Compensation is provided on a per-visit basis within a Humana/CenterWell Home Health context; position supports both full-time and part-time schedules at home-based care settings.
Required Qualifications
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Current and unrestricted Physical Therapy license
- Minimum of one year physical therapy experience preferred
