Job Summary
Supervise and manage all aspects of patient care, including LVN and HHA supervision, while coordinating visit distribution among disciplines and initiating referrals to ancillary services. Lead weekly interdisciplinary case conferences, develop care plans, and establish social action plans to address medically related social and emotional needs. Facilitate discharge planning with the clinical team and physician, provide crisis support to patients and families, and complete all required clinical documentation in accordance with state and federal regulations. Ensure patient safety and enforce abuse prevention policies. Requires RN licensure, two years of home health experience, and reliable transportation.

- Ability to read, write, and speak the English language
- Reliable transportation
- Valid and current auto liability insurance
Desired Qualifications
- 2 years of experience in Home Health preferred
- HomeCare HomeBase experience preferred
- Good communication skills
