Job Summary

Conduct comprehensive health assessments, diagnose acute and chronic illnesses, and establish care plans with short-term and long-term goals for Provider Partner Health Plan members in skilled nursing facilities. Collaborate with primary care physicians to deliver preventive care, order and interpret diagnostic tests, prescribe medications, and refer to specialists as appropriate. Communicate effectively with interdisciplinary staff, allied health providers, family members, and physicians during transitions of care to avoid fragmentation and unnecessary testing. Maintain autonomy in clinical decision-making while adhering to state scope of practice and PPHP standards of documentation. Requires current NP licensure, DEA/CDS credentials, BCLS certification, and minimum three years of clinical experience. Skilled nursing facility and case management experience preferred.

Required Qualifications

  • Clinical competence and understanding of evidenced-based practice
  • Proficient in computer skills- confidence in learning and using unfamiliar computer programs and electronic devices. Ability to export data, create reports and relay results
  • Minimum of 3 years clinical care experience
  • Currently licensed as an Acute, Adult/Gerontological or Family Nurse Practitioner in the state of practice
  • Meets the Provider Partner Health Plan credentialing requirements
  • Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice)
  • Basic Cardiac Life Support certification
  • Current unrestricted RN license for the state in which the Nurse Practitioner is practicing
  • Valid driver's license and car insurance with reliable transportation

Desired Qualifications

  • Skilled Nursing Facility/Long Term Care experience preferred
  • Case management experience preferred
  • Knowledge of current standard medical practices and experience with Medicare/Medicaid
  • Detailed knowledge of I-SNP MOC and I-SNP population
  • Proficiency in nursing/medical databases and Microsoft programs
  • Positive and enthusiastic attitude
  • This role requires independent thinking, autonomy and ability to problem solve and make decisions
