FT Nurse Practitioner- SNF/ALF Eagle Pass

Nurse Practitioner in an ISNP program delivering coordinated, comprehensive care to PPHP members in designated skilled nursing facilities (Eagle Pass, TX). Works with the member’s primary care physician to develop preventive and timely intervention care plans, serves as the medical liaison to the interdisciplinary team, communicates with families and staff, and orders/tests, prescribes medications, and refers to specialists as appropriate. Responsibilities include autonomous comprehensive health assessments, adherence to PPHP documentation/communication standards, collaboration on care plans emphasizing prevention and early detection, and supporting transitions of care. Requires strong clinical competence, proficiency with electronic records, case management experience, Medicare/Medicaid knowledge, and licensure with DEA/CDS where required, plus BLS and a valid RN license. Prolonged emphasis on elderly care, strong interpersonal/communication skills, organizational ability, and autonomous decision-making in the skilled nursing/long-term care setting.
