Job Summary
Mental Health Case Manager responsible for thorough psychosocial assessment, developing and implementing Personal Service Plans using trauma-informed approaches, motivational interviewing, and a range of counselling techniques to support individuals experiencing homelessness or at risk of homelessness. Responsibilities include interviewing clients, collaborating with community and governmental partners, coordinating referrals to housing and services, tracking progress, crisis intervention, and providing variations of both individual and group support. Requires degree in social work or related field, 2 years of relevant experience, and certifications (Emergency First Aid, MANDT), plus a BC driver’s license and access to reliable transportation. Unionized role with standard caregiving duties, documentation, and on-call coverage as needed. Compensation starts at $40.03 per hour with benefits; wage range CAD 40.03 - 50.59 per hour, annualized to USD values in this posting.
Required Qualifications
- Degree in social work or related human/social services field
- 2 years’ experience in a similar setting with mental health or addictions focus
- Current Emergency First Aid Certificate
- Valid MANDT certification
- Valid BC driver’s license
- Personal transportation in good repair and insured for business use
- Criminal Records Check
