Job Summary
Deliver personal care and minimal homemaker services to hospice patients in home, assisted living, or long-term care facilities while maintaining a safe and clean environment. Participate in the development of policies and procedures guiding hospice care delivery. Ensure all services align with Hosparus Health policies and the CNA plan of care. Requires CNA certification, valid driver's license, and one year of home health aide experience preferred. Schedule includes Monday-Friday 8:30am-5:00pm with on-call requirements.
Required Qualifications
- CNA certification required in the state hired to work, or an active LPN or RN license
- Valid Driver's License
- Auto Insurance coverage
Desired Qualifications
- High school diploma preferred
- One year home health aide or nursing assistant experience preferred
- Use of technology-based tools, including electronic medical record, strongly preferred
