Job Summary
Conduct personal care, medication management, and crisis support for individuals with disabilities residing in the group home. Ensure compliance with agency policies, 6100/6400 regulations, and Individual Service Plans while documenting outcomes in the electronic health record. Assist with daily living activities such as bathing, toileting, and feeding; perform housekeeping duties including vacuuming, laundry, and shopping. Act as an advocate for community integration, provide emotional support, and encourage independence through skill-building and respectful communication. Complete mandatory annual training, attend staff meetings, and report safety issues immediately. Work evenings, overnights, weekends, and holidays; physically lift 40-100 pounds and endure repetitive motions. Maintain Medication Administration Certification and pass a pre-employment physical with a TB test.
Required Qualifications
- Valid driver's license
- High school diploma or GED equivalent
- Experience in the field
- No criminal record
- Must be a U.S. citizen
- Must be able to physically lift 40-100 pounds unassisted
- Must be able to physically lift 100 pounds or more assisted
- Must be able to reach above head and below knees
- Must be able to endure repetitive motions with right and left hands
- Must be able to reach out as to pull and push a wheelchair
- Must be able to make twisting and bending motions at the waist
- Must have good reaction response to protect personal space
- Must have a working knowledge of all agency policies and procedures
- Must ensure compliance with 6100 and 6400 regulations along with updated ODP requirements
- Must complete at least 24 hours of training annually
- Must maintain Medication Administration Certification
- Must be able to identify work safety issues, injuries, and report immediately
- Must maintain and adhere to ethics of confidentiality
- Must be willing to teach individuals skills to become independent
- Must be willing to get a pre-employment physical
- Must be willing to get a physical with a mantoux TB test
Desired Qualifications
- Willingness to utilize/learn computers for daily documentation, training, etc.
