Job Summary

Supervise and guide individuals with disabilities in a group home by managing personal care, medication administration, and crisis management. Implement Individual Service Plans, document outcomes accurately, and ensure compliance with agency policies, 6100/6400 regulations, and ODP requirements. Perform housekeeping duties, assist with daily living activities, and advocate for community integration while modeling everyday life philosophy. Provide problem-solving support, encourage independence, and maintain confidentiality. Complete at least 24 hours of annual training, attend staff meetings, and utilize electronic health records for documentation. Work evenings, weekends, and holidays; physically lift up to 100 pounds and respond to challenging behaviors.

Required Qualifications

  • Valid driver's license
  • High school diploma or GED equivalent
  • Experience in the field
  • No criminal record
  • Must be a U.S. citizen
  • Must be able to physically lift 40-100 pounds unassisted
  • Must be able to physically lift 100 pounds or more assisted
  • Must be able to reach above head and below knees
  • Must be able to endure repetitive motions with right and left hands
  • Must be able to reach out as to pull and push a wheelchair
  • Must be able to make twisting and bending motions at the waist
  • Must have good reaction response to protect personal space
  • Must have a working knowledge of all agency policies and procedures
  • Must ensure compliance with 6100 and 6400 regulations along with updated ODP requirements
  • Must complete at least 24 hours of training annually
  • Must maintain Medication Administration Certification
  • Must be able to identify work safety issues, injuries, and report immediately
  • Must maintain and adhere to ethics of confidentiality
  • Must be willing to teach individuals skills to become independent
  • Must be willing to utilize/learn computers for daily documentation, training, etc.
  • Must be able to elevate incidents identified in the Incident Management Bulletin to a point person
  • Must be able to effectively and timely communicate with immediate supervisor
  • Must be able to identify individual's strengths, needs, and goals
  • Must be able to ensure implementation of Individual Service Plans (ISPs) and document outcomes accurately and timely
  • Must be able to complete required documentation in agency approved electronic health record
  • Must be able to perform housekeeping duties such as vacuuming, shopping, laundry, and mopping
  • Must be able to assist or perform aspects of personal care such as tooth-brushing, toileting, bathing, clothing, and feeding
  • Must be able to provide assistance with individual's emotional issues and provide support, education, and solutions
  • Must be able to act as an advocate for individuals in regard to community relationships
  • Must be able to provide opportunities for individuals to integrate and support them within their community
  • Must be able to encourage and respect choices made by individuals
  • Must be able to use positive and respectful language with supervisor, co-workers, and individuals
  • Must be able to utilize and model everyday life philosophy in supporting individuals
  • Must be able to perform any and all related tasks as assigned by the immediate supervisor
  • Must be available for evenings, overnights, weekends, and holidays
  • Must be able to work with individuals who have an intellectual disability and co-occurring mental health disorders
  • Must be willing to endure exposure to individuals who have the potential to be carrying a communicable disease
  • Must be able to get a pre-employment physical
  • Must be able to get a physical with a mantoux TB test
