Job Summary
Conduct case management services to assist Program Participants in achieving interim steps to self-sufficiency by helping them write service plans, set goals, and access community agencies for specialized assistance. Provide counseling in life skills areas including parenting, employment, education, and financial management while establishing effective relationships with families through in-depth discussions to identify needs and impediments to progress. Evaluate participants for motivation levels, maintain liaison with various agencies for referrals, and prepare assessments, case records, and activity reports. Requires bi-lingual English/Spanish proficiency and a bachelor's degree in social services or equivalent experience.
Required Qualifications
- Bi-lingual English/Spanish
- Graduation from a regionally accredited or NYS registered college or university with a bachelor's degree in psychology, sociology, social services, human services, education or a closely related field with similar course curriculum
- Graduation from high school or possession of a high school equivalency diploma and four years of full-time paid experience, or its part-time or verifiable volunteer equivalent, working in human services in a capacity involving the provision of direct services to clientele
- An equivalent combination of training and experience as defined by the limits described in the 2 bullet points above
