Job Summary
Service Coordinator II for the FSP Housing First team who conducts intensive case management, outreach, and housing placement for adults experiencing chronic homelessness and serious mental health issues. Provides comprehensive mental health services, medication management, crisis intervention, and coordination of care using recovery-oriented models (Housing First and Harm Reduction). Conducts ANSA assessments, develops and monitors service plans, connects clients to clinical resources, ensures eligibility and lease-up procedures, and collaborates with co-service providers. Works under supervision in a field and office-based setting, participates in 24/7 on-call coverage, attends staff meetings, and maintains electronic health records and billing duties. Requires Masters-level education and licensure-eligibility, with experience in homeless services and knowledge of CA BC licensing requirements.
Required Qualifications
- Masters in Counseling, Psychology, or Social Work
- licensed-eligible Interns (AMFT, ACSW, APCC) and registered with the CA BBS
- Minimum of 2 years of experience working with homeless populations and/or those diagnosed with a mental illness in a community setting desirable
